How to Submit an Access to Information or Privacy Request

Access to Information Requests

Choose one of the options listed below to receive records held by the Office of the Veterans Ombudsman.

General information about Access to Information requests:

  • A $5.00 application fee is required for correspondence to be treated as a request under the Act (must be a cheque or money order made payable to the "Receiver General for Canada").

  • The Office has 30 calendar days to respond to an Access to Information request. However, the Act does allow for extensions beyond the 30 calendar days, and you will be notified in writing of any extension.

  • Mail your completed letter or form to the address below.

  • Refer to the list of Access to Information and Privacy Coordinators to request information held by other government institutions.

Privacy Requests

Choose one of the options listed below to receive your personal information that is under the control of the Office of the Veterans Ombudsman.

Option 1:

  • Send a dated and signed letter stating that you are seeking your records under the Privacy Act.

  • Provide enough detail to enable us to locate your records.

Option 2:

General information about Privacy requests:

  • There is no fee for a Privacy request.

  • The Office has 30 calendar days to respond to a Privacy request. However, the Act does allow for extensions beyond the 30 calendar days, and you will be notified in writing of any extension.

  • Mail your completed letter or form to the address below.

  • Refer to the list of Access to Information and Privacy Coordinators to request information held by other government institutions.
